Alcohol Risk Management *

If alcohol will be served at your event you must abide by the terms and conditions of the Elliot Lake Alcohol Risk Management Policy. In addition, you are required to obtain a Special Occasion Permit (SOP) and liquor liability insurance for your event.

If food is being prepared and served to the public you are required to obtain a Food Handling Permit from Algoma Public Heath. A copy of the permit must be provided to the City of Elliot Lake before your rental.

Clean up *

Renters are responsible for cleaning the kitchen after use. All surfaces, equipment, and utensils must be cleaned are returned to their original condition before leaving.

You are responsible for the setup and take down of all decorations. Please note: we do not allow the following - helium balloons, open flames, confetti, glitter or silly string. Additional cleaning charges will be applied if decorations are left up.

I will provide my own sound/lighting technician and equipment. I would like to request the City to provide basic sound and lighting support. I understand that City staff availability is limited and that this service will incur additional costs. I do not require sound or lighting.

Insurance *

All Collins Hall users will be prohibited to use the facility unless either a Certificate of Commercial Liability insurance is provided to the City of Elliot Lake or a Certificate of Commercial Liability insurance is purchased through the City of Elliot Lake’s Facility User Insurance program.

General liability insurance coverage for a limit of no less than $5,000,000 per occurrence. Coverage shall include bodily injury, personal injury, property damage and contain a cross liability clause. The vendor must name the City of Elliot Lake as additional insured but only with respect to your operations. A certificate of insurance signed by an authorized representative MUST be submitted with the agreement. If you are NOT a business, commercial entity, an instructor or a professional therapist and you do not have insurance, you must purchase general commercial liability insurance through the City of Elliot Lake for this event.
