All students registering for in-person classes are asked to please review our Wellness Policy. Note that PHAME no longer requires proof of Covid vaccination, however we strongly encourage our community to be vaccinated for Covid and the flu.
Student Forms
The Student Information form collects important information about your identity, how to contact you, accommodations that will help you participate at PHAME, and what to do in case of an emergency.
Deadline: As soon as possible. PHAME cannot process your registration until after we receive this form.
All students must have a signed copy of the Student Participation Agreement on file in order to participate in PHAME classes and programs.
Deadline: The Monday that falls one week before the term begins.
The Tuition Assistance Request Form collects information about your financial situation, and helps PHAME determine how much Tuition Assistance to award you. If you are able to pay full price for PHAME classes, you do not need to fill out this form.

If registration is currently open for the upcoming term—and if you haven’t already registered for classes—fill out our Registration Form. This form tells PHAME what classes you’d like to take.
Deadline: New students get first dibs on classes. If registration is open, we’ll process your registration form as soon as we receive it. So, the sooner you submit it, the better.
To keep PHAME a safe and respectful place, we expect everyone to follow PHAME’s Code of Conduct. This applies when you’re in class (both online and in person), on the PHAME campus, at rehearsals, community events, performances, or any other PHAME-related activities.
The Code of Conduct applies to PHAME students and staff, and to any support people or family members who come with students.
