Waiver and Release of Liability:

In consideration of being allowed to participate in the Taekwondo Manitoba Gold Tie Program (the “Program”) and any associated activities, events, and training (collectively, the “Activities”), I, the undersigned participant (or the undersigned parent/legal guardian on behalf of the participant), hereby acknowledge, understand, and agree to the following:

Understanding of Risks: I understand that participation in taekwondo involves physical activity and contact, and that there are inherent risks of injury, including but not limited to sprains, strains, bruises, fractures, dislocations, head injuries, paralysis, and even death. I voluntarily assume all such risks associated with my (or my child’s) participation in the Program and Activities.

Release of Liability: I, for myself, my heirs, executors, administrators, and assigns, hereby waive, release, and forever discharge any and all rights, claims, actions, causes of action, demands, damages, costs, losses, and expenses of any kind whatsoever, whether known or unknown, foreseen or unforeseen, which I may have or may hereafter accrue against Taekwondo Manitoba, its members, directors, officers, employees, volunteers, instructors, agents, representatives, and sponsors (collectively, the “Released Parties”), arising out of or in any way connected with my (or my child’s) participation in the Program and Activities, including but not limited to any injury, illness, or death resulting therefrom, even if such injury, illness, or death is caused by the negligence or other fault of the Released Parties.

Assumption of Responsibility: I understand and agree that the Released Parties are not responsible for any injuries or damages sustained during travel to or from any Program or Activity location. I am solely responsible for ensuring that I (or my child) am physically and mentally fit to participate in the Program and Activities.

Compliance with Rules and Instructions: I agree to abide by all rules, regulations, and instructions provided by Taekwondo Manitoba and its representatives during my (or my child’s) participation in the Program and Activities.

Medical Treatment: I hereby consent to the provision of medical treatment to me (or my child) in the event of an injury or illness during the Program or Activities, as deemed necessary by Taekwondo Manitoba or its representatives. I understand that I am responsible for any and all costs associated with such medical treatment.

Photo Release:

I hereby grant to Taekwondo Manitoba the irrevocable right and permission to photograph, videotape, and otherwise record me (or my child) during my (or my child’s) participation in the Program and Activities. I further grant to Taekwondo Manitoba the right to use, reproduce, publish, distribute, and display such photographs, videos, and recordings in any and all media, including but not limited to websites, social media, promotional materials, and publications, for any purpose whatsoever, including but not limited to advertising, marketing, and promotion of Taekwondo Manitoba and its programs, without any further consent from or payment to me (or my child). I understand that my (or my child’s) name and likeness may be used in connection with such materials. I waive any right to inspect or approve the finished product or the use to which it may be applied.

Acknowledgement:

I have carefully read this Waiver and Release of Liability and Photo Release, fully understand its terms, and sign it voluntarily. I am aware that by signing this document, I am giving up certain legal rights.
