2025-2026 USA Curling Membership Dues Invoice Request

Please use the following form to upload your roster for USA Curling and request your 2025-2026 Membership Dues Invoice. If your roster changes throughout the year, this form should be used to report additional members to be added to your roster.

Please use the provided template for your roster and fill out as many fields as possible. This will help reduce the number of duplicate accounts created. If you have not received the template, or if you have questions about formatting or uploading it, contact Lauren Rich (lauren.rich@usacurling.org) for additional information.

You should receive a confirmation email to the address provided in the form when you submit the roster. If you do not receive this email, contact Lauren.

In order to be eligible for the instant rebate of $5/member (unlimited for dedicated ice clubs, limited to $150 for arena clubs), roster submissions must be received through this form by 11:59 PM CST on November 30th, 2025.
