QUESTO RECREATIONAL CLASSES

GENERAL

Training uniform: Black t-shirt, black shorts or leggings, white socks, hair neatly tied.

Training equipment: Provided by Questo.

Arrival time: Please arrive 5 minutes before class. 

Holidays: No classes on official public or school holidays.

Parents are asked to assist the coach with rolling out and/ or putting away the carpets before and/ or after class.

PAYMENT AND CANCELLATION POLICY

• Registration (annual membership and FGQ affiliation): $90.
• Non-refundable after the first week of training.
• Questo fees are an annual (Oct-June) tuition divided into 9 equal installments (not a monthly subscription fee) and are prorated for late joiners.
• Early Cancellation: A fee applies, equal to the lesser of $50 or 10% of the unused portion of services.
• NSF Fee: A $10 fee will be charged for any returned e-cheque due to payment failure.
• Non-Payment: Questo reserves the right to suspend a gymnast from training if registration or training fees are unpaid.
• Illness Credit: Credit may be granted in cases of prolonged illness (more than 14 days). Written notice must be sent to the administrator at admin@questo.ca.
• Missed Classes: No refunds for missed classes.
• Cancelled Classes: Classes cancelled by Questo will be rescheduled. Additional or replacement classes will be scheduled from December 15 to 20 and from June 15 to 20.

WAIVER FORM

In consideration of the participation of my minor child in Questo Rhythmic Gymnastics classes, I confirm that:

1. I have full legal responsibility for decisions regarding my minor child.
2. Her participation in this program is voluntary.
3. My minor child has undergone a medical examination within the 12 months prior to registration, and no medical condition exists that restricts her participation in this program.

​I UNDERSTAND AND AGREE, on behalf of my minor child, my heirs, assigns, representatives, and next of kin, that by signing this document I accept the following:

1. Assumption of Risk –I acknowledge that participation in sports activities involves inherent risks, including injury, disability, or death, which may result from my child’s own actions, the actions of other participants, the use of equipment or facilities, or the negligence of organizers or coaches.
2. Release of Liability – To the fullest extent permitted by Quebec law, I release and discharge Questo, its directors, officers, coaches, employees, volunteers, sponsors, facility owners, and all persons associated with the program (“the RELEASEES”) from any liability for injury, disability, death, or property damage resulting from participation, except in cases of intentional gross negligence or intentional fault.
3. No Lawsuits – I agree not to pursue legal action against the RELEASEES for ordinary negligence relating to participation in the program.
4. Medical Information – I confirm that my child medically and physically fits to participate, and I agree to disclose any relevant medical conditions to the coaches.
5. Indemnification – I agree to indemnify and hold harmless the RELEASEES from any claims, costs, or damages arising from my child’s participation, except where caused by intentional gross negligence or intentional fault of the RELEASEES.

I have read and understood this document. I acknowledge that by signing, I accept the risks of participation and agree to limit certain legal rights of myself and/or my minor child, except in cases of intentional gross negligence or intentional fault.
