Membership Registration Form

Step 1: Fill out the registration form and enter your contact information. Select your club/program from the dropdown list and use a general club or program email address whenever possible. Submit the form once all information has been completed.

Uploading Your Members Step-by-Step Tutorial

 

Follow these simple steps to upload your members for the season.

Step 1 – Membership Registration

  • Open the link to fill out a Membership Registration form
  • Enter your contact information and select your club/program.
  • Submit the form.

Note: Your Provincial Lead will review your request before access is granted. If your club or program does not appear in the list, contact your Provincial Lead.

Step 2 – Connect Your Account

  • Return to the same page.
  • Enter the email address you used on your access request.
  • Request a connection code.
  • Enter the code sent to your email.
  • Once your access has been approved, you'll see a Member Import tab.

Step 3 – Import Your Members

  • Open the Member Import tab.
  • Click Filled forms All and select the 2026-27 Membership Form (Import)
  • Click the red button labeled Import.
  • Download the provided spreadsheet (schema/template).
  • Enter your member(s) information into the spreadsheet.
  • Copy the spreadsheet data and paste it into the import window.
  • Click Import
  • Correct any errors (such as missing fields or incorrectly formatted dates).

Helpful Tips

  • Use each member's own email address whenever possible so they can receive their membership information.
  • A member can have up to three affiliations (for example: Athlete, Judge, Coach). If a member requires more than three affiliations, submit an additional import.
  • You can import one member or hundreds at a time.

Step 4 – Member Completes Their Waiver or Coaching Requirements form

 

After your import:

  • Each member will receive an email asking them to complete their required waiver.  When importing coaches they will receive an additional email so they can upload their required information.
  • If the member is a minor, a parent or guardian will complete the waiver on their behalf.

Step 5 – Receive Your Invoice

Once your Provincial organization has reviewed and approved your imported members:

  • An invoice will be generated.
  • The individual who first requested club access will receive the invoice notification.
  • Payment options (such as credit card, e-transfer, cheque, etc.) will depend on your Provincial organization's setup.

Uploading Additional Members

Whenever you need to add more members:

  • Return to the Member Import tab.
  • Repeat the import process.
  • Your import history and invoices will remain available for future reference.
