Form questions: Standard blocks

This article is part of the Form question reference. It covers the blocks of the Add a new block picker's Standard questions tab. The reference index explains how blocks are added, the form types and the options shared by most questions.

Standard questions

The picker text reads: “Collect any number of information from your respondents. In the Respondents tab, click on Visible columns to view answers in the table.”

Yes/No

Collects a single choice between Yes and No. It creates a Radio buttons question with those two values and Required on, and opens the Radio buttons editor. Available in every form type.

Input

Collects free text. Available in every form type.

Option Default Effect
Multiple lines No Shows a text area instead of a single line.
Rich text editor No Only with Multiple lines. The respondent gets formatting controls.

Maximum word count Off, 100 Limits the number of words. A counter shows under the field.
Maximum character count Off, 100 Limits the number of characters.
Default Empty Prefilled text.
Description, Placeholder, Invoice/Summary, Attendance, Client account — See the shared options.

When the label contains a word such as “number”, “amount”, “price” or “how many”, the editor proposes “Seems you are asking for a number. Switch to a Number question instead?” with a Switch button.

Number

Collects a numeric value. Available in every form type.

Option Default Effect
Decimal places 0 Number of decimals accepted.
Default Empty Prefilled value.
Minimum, Maximum — “Leave blank for no restriction.” Empty Validation bounds.

Number preset by admin

Label in the picker: Number preset by admin. “Allows you to preset and modify a number value. The respondent will not be able to change the value. Useful to preset the salary in an employment contract.” Not available in Checkout, Client profile or Waitlist forms.

Rating

Collects a star rating. Not available in Checkout or Waitlist forms.

Notification based on selection

“Allow the respondent to select an option. Based on the answer, send an email notification with the filled form PDF attached. Email addresses are not visible to the respondent.” Not available in Waitlist or Client profile forms.

Many inputs on the same line

Collects several short answers on one row. Not available in Checkout, Client profile or Waitlist forms.

Formula

Calculates a value from other questions. Not available in Waitlist, Client profile or Client account forms. Also listed under E-Commerce and Expense claims.

Collects one choice from a list. Available in every form type.

Radio buttons

Collects one choice. Available in every form type.

Option Default Effect

Option details — “Provide an input field to enter details” No Adds a text field labelled Please specify to a choice.
Fail value Empty “Specify what value is considered a fail. Set to empty not to count fail answers for this question. Used in Attendance tracking.”

Multi radio buttons

Radio buttons organized as parent options with sub-options. Available in every form type.

Radio grid

A grid of rows with one radio choice per row. Not available in Checkout or Waitlist forms.

Weight Off (weights 1, 2) Shows a numeric weight per column for scoring.
Keep data from previous answers No “The answer will be saved and other respondents will be able to see the previous answer.”
Create placeholders for each row No Makes each row's answer available as a merge field named {Question label: Row label}. Renaming or removing the question or a row blanks existing placeholders.
Row spacing 12 px Extra space between rows.

Checkboxes

Collects several choices. Available in every form type.

Locked No Preset values the respondent cannot change.
Collapsible when empty → Expand label No Hides the choices behind a button until the respondent expands them. “Option not available when the question is required or locked”.

When a Checkboxes question is linked to communication tags, the editor shows “These choices come from Tags in Organization settings. The question and description can be edited here.”

Multi checkboxes

Checkboxes organized as parent options with sub-options; same editor as Multi radio buttons, with multiple selection. Available in every form type.

Checkbox grid

A grid with any number of checks per row. Not available in Checkout, Client profile or Waitlist forms.

Number grid

A grid of numeric fields. Not available in Checkout, Client profile or Waitlist forms. Options: Rows, Columns, Row spacing, Keep data from previous answers, Required (default Yes), Hidden, Description, Attendance.

Ranking

The respondent orders a list. Not available in Checkout, Client profile or Waitlist forms.

Subscribe to our newsletter

A consent checkbox that subscribes the respondent. Not available in Checkout, Client profile or Waitlist forms.

Bookings waitlist

Shown only when the organization has bookings; not in Waitlist, Client profile or Client account forms. The respondent asks to be told about free booking slots. “This question is linked to a Booking availability notification automation. Shared availability settings are synchronized in both directions. Saving a new question creates the linked automation automatically.”

Communication → Allow these communication types Email Email, or email and SMS.
Waitlist automation → Send automatic availability messages On Message when automation is turned off defaults to “The wait list is turned off.”
Communication template Empty The message must contain a button to the microsite bookings page; a block with free spots is appended.

Grant selector

Shown only on grant follow-up forms of organizations with grants. “This question allows the respondent to choose one of their grants. They must first be connected.” Options: Hidden, Description; the answer is included on the invoice by default.

Conditional branching

A radio (or checkbox) question whose choices show or hide other questions. Not available in Client profile or Waitlist forms. Full guide: Using conditional branching.

Rules enforced by the editor: a branched question must be placed after its branching question; a page break and the Account owner block cannot be branched. The “&” character is not allowed in option values.

Page break

Splits the form into pages. “Questions after this page break will be shown on a separate page.” Not available in Checkout, Client profile or Waitlist forms. Its only field is the Label, used as the page title.

Question filters

A multi-filter block that shows questions matching the selected filters; it does not work with page breaks. Shown only for a few organizations and marked PREMIUM. Options: filter names, each with options that list Show questions.
