Add a U.S. Social Security Number question to a form

Activity Messenger has a dedicated Social Security Number (USA) question for U.S. SSNs. Use this encrypted question type instead of a regular text question when your form needs to collect an SSN. It is separate from the Canadian Social Insurance Number (SIN) and RL-24 payer workflow.

Add the question

  1. Open your form in the form editor and add a new block.
  2. Open Encrypted, then look under Confidential.
  3. Select Social Security Number (USA).
  4. Set the Label and, if useful, a Description explaining what the question is for.
  5. Turn Required on if respondents must answer it, or leave it off to make the answer optional.
  6. Leave Hidden off so clients can see and complete the question. Turning it on makes the question visible only to administrators; it is not necessary to turn it on to encrypt the answer.
  7. Click Create for a new question, or Save when editing an existing question. Preview the form as a respondent to check its visibility and instructions.

What clients enter

The question expects nine numeric digits and displays them in groups of three, two and four, such as 123 45 6789 (format illustration only). Enter the digits without hyphens. If the question is optional, respondents may leave it blank.

The validation checks the expected format. It does not establish that an SSN was issued or belongs to the respondent.

Confidentiality and visibility

SSN is an encrypted question type. The editor’s notice explains that the answer is encrypted and visible only to account owners and administrators. This answer-access rule is separate from Hidden, which controls whether clients see the question on the form.

If you cannot find the question

The Encrypted section is not offered in checkout, waitlist or profile form editors. Use an ordinary form where that section is available.

The section may display a Premium badge. If it is unavailable in your organization’s editor, ask support to check your form type and feature availability. Do not substitute a regular text field for the dedicated SSN question.

For Canadian SIN collection associated with RL-24 slips, use the applicable Canadian workflow instead of this U.S. SSN question.
