Form questions: Upload and Electronic signature blocks

Upload

“Allow respondents to upload documents, images and videos.” Not on Waitlist or Client profile forms. “All uploaded files are stored in encrypted storage and use unique, hard-to-guess links. Standard uploads have a permanent public URL. For documents that require stricter access control, choose Private document.”

Person photo

An image upload preset with Person photo on (“Only pictures will be allowed - no PDF.”) and the description “Please upload a passport-like portrait photo”.

Upload an image or PDF

Accepts JPEG, PNG, GIF, WebP and PDF.

Upload a video

MP4, QuickTime, 3GPP, WebM and M4V. Same options as images.

Upload an audio file

MP3, WAV, FLAC and M4A. Same options as images.

Upload documents (Excel, Word, PowerPoint, PDF, TXT)

Same options as images. The editor warns: “Excel, Word and PowerPoint documents could contain viruses or malicious code … Do not make this form publicly available on your website.”

Upload private documents

“Private documents are stored in a private, non-public location. They can only be accessed by authorized admins via time-limited links (10 minutes). Allowed file types are: Images, PDF, Excel, Word, PowerPoint, or TXT.” Options: Allow multiple, Required, Hidden. “The PDF will not include links to private documents.”

Electronic signature

“Capture consent as an electronic signature, or by using a checkbox. Activity Messenger stores the timestamp and IP address of the respondent.” Not on Waitlist or Client profile forms. Guide: Waivers: collect consent and electronic signatures with a form.

Acceptance

A checkbox under a block of terms.

Electronic signature

The respondent can Draw the signature or type their name (Your Name...).

Administrator signature

A signature an administrator draws (Draw) or types (Your Name...) in the editor and that is printed with the response. Options: Label, the signature.
