Form questions: Encrypted, Hidden and Samples blocks

Encrypted
“Sensitive answers will be encrypted and only visible by users with the Account owner and Administrator permission.” Not on Checkout, Waitlist or Client profile forms. Marked PREMIUM on starter and demo plans.
Encrypted
Free text stored encrypted: “The answer to this question will be encrypted and only visible to account owners and administrators.” Options: Multiple lines, Required, Hidden, Description.
Bank account information
Encrypted bank account details (placeholder 12345 123 123456), with optional Instructions shown to the respondent. Options: Show instructions by default, Add imported bank account as payment method (“When admins import this form, a valid bank account answer will add or replace the client AFT bank account on file.”), Required, Hidden.
Social Insurance Number (Canada)
Encrypted field with placeholder 123 456 789. Options: Required, Hidden, Description.
Social Security Number (USA)
Same options as the SIN block.
Health insurance card
Encrypted health card number. Options: Required, Hidden, Description.
RL-24 payers
Shown for one organization with RL-24 enabled and an eligible class on the form. Collects who paid childcare expenses, encrypted, with Automatic second-payer invitation.
Hidden
“These questions will only be visible to administrators. Clients will not see them. Use these to track internal processes and workflows.” Not on Checkout or Waitlist forms.
Admin note
Free text for administrators. Options: Label, Description, Invoice/Summary, Attendance.
Dropdown, Rating and Date/Time (admin only)
The same blocks as in Standard questions, created with Hidden on so that only administrators see them.
Approve and notify
Picker label: Approve and notify; default label Approval.

Assign skills
“With this question, skills can only be assigned to the account owner. They cannot be unassigned.” Options: Skills (values), Inline, Show buttons to select all or clear all. See Students and skills.
Respect group check
Shown when the organization has a Respect Group account: “This question will run a background check on the Respondent via Respect in Sport Activity Leader”. Option: Available Information (expiry date, certificate number, validity).
Unique ID
Shown when enabled for the organization. “Generates and associates to the respondant a unique ID in the form "V####A" which starts at 0001 and will be incremented and unique per email address.” Options: Label, format, Attendance. Always admin-only.
Samples
“Create your own sample questions from existing ones. In your form, click on the menu of a question and choose "Create sample".” The Samples tab lists reusable questions with Modify and Delete; adding one copies it into the form. Samples of invoice-item types appear on Payment forms only.
