Form question reference

This page is the index of every block you can add in the form editor, grouped the way the Add a new block picker groups them. The linked articles state, for each block, what it collects, which form types offer it, and every option with its on-screen label, default and effect. The reference is split into one article per Add a new block picker tab. This page keeps the shared rules (how blocks are added, form types, shared options) and links every block to the article that describes it.

How blocks are added

Open the form, then the Form tab. Click + between two blocks, or open a question's three-dot menu and choose Add a question → Before or Add a question → After. The Add a new block picker opens with vertical tabs: Standard questions, E-Commerce (payment forms only), Content, Classes/memberships (forms without payment only), Date/Time, People, Upload, Electronic signature, Expense claims, Encrypted, Hidden and Samples. Clicking a block opens its editor window; Create adds it to the form, Cancel discards it. Reopen a block with Edit in its three-dot menu; the same menu offers Duplicate, Create sample, Translate and Remove. Removing a question deletes its answers: “Please confirm your intention to remove this question. You will lose all associated responses.”

The first block of a form is the Account owner block. It is not in the picker and cannot be removed; it identifies the respondent. Its fields are described under Account owner.

Form types and availability

The picker hides blocks that do not apply to the form being edited. This page uses these names:

Form type What it is
Generic A form created under Forms with no Payment step.
Payment A form of type payment created from E-Commerce; it generates an invoice.
Checkout The checkout form of an organization (type checkout), used for contracts and payment pages.
Client account A form shown as a tab in client accounts.
Staff A staff form (type staff).
Client profile The organization's profile editor under Organization settings → Profile fields.
Waitlist The organization's waitlist form.

Unless a section says otherwise, a block is available in Generic, Payment, Checkout, Client account and Staff forms. A client-account form rejects these types on save: Class, Multiple classes, Event with tickets, Membership, Mass registration, Question filters, Formula, Bookings waitlist, Time track, Check-in/Check-out, Week availability, Expiration date, Participant/member/student, Other parent, Contacts, Participants, Person select, Age category, Affiliation, Multiple age categories, Highest amount of Age category, Relay and the expense blocks.

Some blocks appear only when a feature is enabled for the organization; each section says so.

Options shared by most questions

Option Default Effect
Label The block name The question text. Required, 200 characters maximum.
Description Empty Rich text shown under the label. Click Show to open the editor.
Required Varies by block The form cannot be submitted without an answer.
Hidden — “Visible only to administrators.” Off Clients never see the question. Administrators see and fill it from the respondents table.
Locked — “Allows you to preset and modify the value. The respondent will not be able to change the value.” Off Available on Input, Dropdown, Date/Time, Date range, Expiration date, Checkboxes and the invoice-item blocks.
Must be unique — “Ensures that no two respondents provide the same answer.” Off Input only.
Placeholder Empty A merge-field name so the answer can be inserted in emails, documents and automations.
Invoice → Include on the invoice and in the notification email sent to administrators. Off On payment forms. Generic forms show Summary → Include in the PDF summary and in the notification email sent to administrators. instead.
Show on the invoice in the client block. Off Payment forms only.
Attendance → Include the answer to this question in the participant details in the attendance list (for classes). Off Makes the answer visible on attendance lists.
Client account → Allow update in the client account Off Shown when client tabs are enabled. Lets the client change this answer from a client tab without reopening the form. The question needs a placeholder and the tab must expose it as a column.

Conditional display is not an option on individual questions. It is done with the Conditional branching block; see Using conditional branching.

Required participant fields from Organization settings

Open Organization settings → Profile fields (the page is also called Client profile). It reads: “Configure the shared profile for clients and staff. Choose the information collected for account owners, participants, and contacts, and where each field is visible.” The Fields section has one tab per group: Account owner, Participants, Contacts. Each tab has a Label, a Description and the field list where every field is a switch plus Optional or Required; Name is always on. Choice fields open a modal: Gender identity choices, Age constraint, Relationship choices, Grade options, Health conditions, Allergies. Custom fields adds organization-wide fields with a Label, Description, Choices, Multiple, Sensitive information or Card option. When client profiles or staff management are enabled, Permissions sets, per field, Visible, Editable or Read only, Staff and attendance visibility; “Required fields must be editable by clients.”

The Settings section holds Client account settings such as Allow clients to add participants/account members, Show occasional participants in the client profile and Occasional participants tab label.

Which question exposes which group:

Profile group Question blocks that use it
Account owner The Account owner block; Person; Company.
Participants Participant/member/student; Children; the participant section of Class, Event with tickets and Membership blocks; Participants in the profile editor.
Contacts Contacts; Other parent; Relay.

The profile is a default, not a live link. In a question editor, Apply default fields → choose Account owner, Participants or Contacts → Apply copies the saved fields into that question: “Apply the fields from the selected client profile group to this question. You can edit those here.” Fields marked Required in the profile arrive as required; you can still change them on the form. If a group has no saved fields, the editor shows “No saved client profile fields are available for this group. Configure them in Client profile, then reopen this question.”

Conditional display

To show a question only after a given answer, add a Conditional branching block and list the questions under each option's Show questions. Multiple classes filters can also list Show questions per option, and the Question filters block does the same for a few organizations. There is no per-question “show if” setting. Common questions

How do I add a membership question? On a payment form, open + → E-Commerce → Membership (on a form without payment, Classes/memberships → Membership), choose the membership, and set Required. Add one Membership block per membership you sell. See Membership.

How do I add an aftercare option? In the Class block, open Add-ons, switch on Class, enable Include form add-ons and keep or rename the After care row with its Price. Switch on Per date to charge per selected day. See Add-ons.

Which block asks for the child's information once for several classes? People → Participant/member/student, with Multiple on when a family registers several children. Then leave Different participant off in each Class block.

Can a question be shown only when another answer is selected? Only through the Conditional branching block; see Conditional display.

Why is a block missing from the picker? The picker depends on the form type (payment, checkout, client account, staff, profile, waitlist) and on organization features such as bookings, grants, references, federation settings or the unique-ID flag. See Form types and availability.

Where do I make a participant field required for every form? Organization settings → Profile fields → Participants, then Apply default fields in each question that should follow it. Existing questions are not updated automatically.
