Form questions: People blocks

This article is part of the Form question reference. It covers the blocks of the Add a new block picker's People tab. The reference index explains how blocks are added, the form types and the options shared by most questions.

People

“People captured in forms are added to the Account Owner's account. Click on Lists, Client accounts to access your database of clients.” Every person block shows a Fields list where each field is a switch with Optional or Required. Apply default fields loads the fields saved under Organization settings → Profile fields for Account owner, Participants or Contacts into the question; see Required participant fields.

Account owner

The fixed first block. It identifies the respondent and is the client account in Lists → Client accounts. Default description on payment forms: “You will receive your receipt by email.”

Option Default Effect
Name, Email Required Cannot be turned off, except on Generic forms where Make anonymous is allowed: “Since the email field is not required, responses can be anonymous. Anonymous respondents will not receive email confirmations.”
Mobile Optional
Company → label, Pronouns, Gender → Gender identity choices, Gender constraint, Date of birth → Age constraint, Relationship, Grade → Grade options, Ranking points, Medical information, Health conditions, Allergies, Other phone, Zip/Postal code, Address → Default country, Address constraint (cities), CC email, Photo Off Each can be Optional or Required.
Form-only custom fields Empty “Add fields needed only on this form. They are not automatically added to the client profile.”
Client account → Allow client to connect Off “Invite the client to connect if they have an account. The session will last 30 days.” Always on for client-account forms.
Allow update in the client account, Attendance Off

Participant/member/student

Picker label: Participant/member/student; on membership forms, Different member. Collects the person being registered when they differ from the account owner, so several Class, Event or Membership blocks ask for the participant once. Only one per form: “Only one Participant/member/student question is allowed per form. To collect multiple participants, enable the Multiple option on that question.” Not available in Client profile or Client account forms.

Participants

Client profile editor only: the Participants group of the profile. Same fields as above, with Allow multiple on.

Other parent

A second guardian. Not available in Client profile; on membership forms it is listed. Fields as for a Person, plus Let this parent choose to appear on the invoice (“This parent’s name, email, mobile number and address will appear in the invoice’s client section only if they check it.”) and Relationship (“For youth forms, ask what is the relationship of this adult with the child.”).

Contacts

Emergency contacts or other adults; Allow multiple is on by default. Fields: Relationship → Relationship choices, Name (required), Email (required), Mobile (optional), Pronouns, Gender, Date of birth, Company, Other phone, Zip/Postal code, Address, Can pickup child, Shared custody, No-contact order, Photo; Also register these contacts as participants in the attendance list. (“This will impact the number of participants in the class. Use with caution.”).

References

Shown when staff references are enabled; not on Checkout, Payment or Client profile forms. “Each reference receives a private link to this form. Their response is linked back to the candidate automatically.” Options: Reference form, Message template, Allow multiple references with Minimum references (1) and Maximum references (3, up to 20), Reference types with a minimum each, fields First name, Last name, Email (required), Mobile, Relationship (optional). Always required.

Children

Several children with Allow multiple on. Fields: Name (required), Gender, Date of birth (optional), Age constraint, Grade, Password to release custody of the child, Pairing with a friend, Email, Mobile, Photo. Not on the Client profile editor. On membership forms, use it for family memberships.

Person

Any other person. Fields as for Account owner without the account options. Not on membership forms.

Company

Collects a company with Name, Email, Mobile, Company, Other phone, Address, Zip/Postal code, CC email, Photo.

Person select

Picker label: Person select. Shown when enabled for the organization; not on Checkout or Client profile forms. The respondent picks people already collected by another form or client tab.

Age category

Choices with an age constraint each. Not on Checkout or Client profile forms. Options: Values (Category 1, Category 2) each with an Age constraint, Price and Product code(SKU); Include prices and product codes(SKU); Required (default Yes); Attendance.

Highest amount of Age category

“Will find the highest amount of all selected Age category prices in this form.” One per form.

Relay

“Ask another person to complete filling the form. The form will be saved and sent to them by email to continue filling the form.” Not on Checkout or Client profile forms. Fields: Name, Mobile and the relay email, with Show relay email as dropdown. Relay settings: Button (Save and send), Email template, Instructions (“Please review and complete this form.”), Relayed person can only answer unanswered questions, Confirmation message.
